Repeat prescriptions-
Please allow 3-4 days for your prescriptions to be organised
For clarity and safety when you need your prescriptions renewed you MUST complete a prescription request form . This ensures that we have an up to date list of your medications. Details of your medications must be written on the prescription request form prior to prescriptions been issued.
Basically we need you to know what your taking so we can assess the safety and correctness of this before issuing you a new script (maybe ask you to redo a blood test or other safety action). So it is very very important to type in each medication, its strength, how many to take per day, how many you need per month and how many months you need
We do not take prescription requests by phone ( too many errors) but we are very happy to take them in writing such as

post to Goatstown Medical Centre, Goatstown, Dublin D14 DY94 (enclose a SAE and we will post back)
On line Form below for online prescription- pay, fill in the boxes and submit
SO DONT FORGET 1. Your info including Date of birth
2. Prescription list- in detail
3. Pay €20 if private- by online payment below
4. Name of your Doctor
5. Name and address (or healthmail address) of your pharmacy and WE CAN EMAIL DIRECT TO THEM
Your prescription will be available four days after your request. If you are unable to pick up your prescription, you may enclose a stamped addressed envelope together with a fully completed repeat medication form and the prescription will be mailed to you.
You may be requested by our receptionist to see the doctor for a medication review before a prescription is renewed.
